About this template

Form is a website template for a physiotherapy clinic, built around two ideas: a drawn body that maps the treatments, and a treatment page whose index stays pinned while the detail scrolls. The demo is a five-person clinic in Singapore with a rehab studio and small classes, and WhatsApp as the way to book.

The home page opens on a one-line drawing of a runner that draws itself as the page loads, with eight hotspots naming the treatments and the statement beside it, then the clinic in three paragraphs beside a photograph of the team, the first visit in four steps, the week's classes as a compact timetable, three patient words about what they got back to, and the address with a map. Treatments is the pinned index: eight treatments fixed on the left, the current one marked, while each scrolls past on the right with its photograph, what to expect, how many sessions it usually takes, the price per session in Singapore dollars, four questions and a book button. Team presents five physiotherapists with qualifications, focus and a personal line, each with a booking link. First visit explains clothing, documents, appointment length, insurance and cancellations. Classes is a weekly grid of five days by four slots with the class, the physio and the spaces, booked by WhatsApp with the class in the message. Prices separates sessions, packages of five and ten, and classes. Contact gives the address, hours, phone, WhatsApp, email, the MRT station and parking.

To make it yours, describe your clinic in a chat with Firevibe: your treatments and their prices, your practitioners with portraits and qualifications, your timetable, your first-visit steps and your contact details; the drawing's hotspots are renamed to your treatments.

Best use cases

Neighbourhood physiotherapy clinic

The team profiles, the first-visit guide and the local directions tell a new patient who they will meet and what happens, with WhatsApp one tap away.

Sports rehabilitation practice

Sports injuries and Running are treatments of their own in the index, and the patient words describe returning to long runs and climbing.

Post-operative rehabilitation

The After surgery treatment covers operation notes, restrictions, milestones and working with the surgeon, with the session range and fee beside it.

Physio-led movement studio

The weekly timetable names each class, its instructor and the group size of four to six, with class packs on the Prices page.

How it works

  1. Use the template

Sign up or log in, and the template's code is copied into a new project of your own. The site is served right away, at its own address.

  2. Change it by chat

Describe what you want different: the words, the images, the pages, the colors. Firevibe edits the code and the preview updates.

  3. Publish

Put the site live on a firevibe.dev address, or connect your own domain.

Features

The drawn body

A single continuous line draws a runner on load; eight hotspots name the treatments and link to them, and a hotspot's part of the line brightens on hover.

Pinned treatment index

On Treatments the eight names stay fixed on the left while the detail scrolls on the right; the mark follows the scroll and a click jumps to a treatment. On phones the index is a sticky strip.

Treatment detail

Each treatment gives what to expect in three paragraphs, the usual number of sessions, the session length, the fee and four questions.

Practitioner booking links

Each of the five profiles carries its own WhatsApp link with the practitioner's name already in the message.

First visit in four steps

From the first message to the exercises to practise at home, with what to wear and bring.

Weekly class grid

Five days by four slots with the class, the physio and the spaces, cells fading in by row.

Price tables

Sessions, packages of five and ten, and classes, each with its conditions, set as a designed page.

WhatsApp booking

Every book button opens WhatsApp with the treatment or class, the day and the time prepared; there is no form and no calendar.

Sticky book bar on phones

A bar with the book button and the phone number stays at the bottom of the screen on a phone.

Arrival information

The Contact page gives a map, two nearby MRT stations with the walk, parking notes and a line about access needs.
